If you're new to Liberated Spirit, welcome! This page is intended to give you a quick overview of everything you need to know before you come to the clinic and what to expect on your first visit.
BOOK AN APPOINTMENT
The easiest way to book an appointment is online via this website. Click the "book now" button on the homepage or the "book an appointment today" button at the bottom of each webpage. Remember to select your preferred provider under "office" and any new patient appointment for "type of service" since it's your first time with us. Please note that appointments will automatically add 15 minutes to each session for practitioner cleanup time. When you book future appointments, please use our patient portal www.onpatient.com. INSURANCE VS. CASH PRICES
ALL NEW CASH AND INSURANCE PATIENTS OWE OUR CASH RATE FOR THEIR FIRST APPOINTMENT This helps us prioritize new patients, as current insurance company staffing shortages have made it cumbersome to verify benefits. For insurance clients, you are welcome to send us your insurance information per below, or bring your insurance ID card to your first appointment. We are happy to verify your benefits after your first appointment and apply any credit to your first and future appointments, or provide a refund upon request. Note, this does not apply to Motor Vehicle Accident or Workers Compensation.
If it’s been more than 12 months since your last appointment, you are considered a new patient.
If we are billing your insurance, please take a photo of the front and back of your insurance card and email them to info@liberatedspirit.com with your date of birth prior to your appointment. As a courtesy for our clients, we will verify your insurance benefits, and call you within two business days to explain how much you will owe on your first and subsequent treatments. NEW PATIENT INTAKE “PAPERWORK”
New patients should expect medical intake paperwork, HIPAA consent and clinic policy forms to fill out and sign. We are a paperless office, so these can be completed via our patient portal www.onpatient.com before your appointment - text your email to our clinic mobile number above and we will send you the link to the portal to create a login - or in the office on the day of your appointment on one of our ipads. On the portal, you can book/edit appointments, fill out new patient forms, upload/download documents such as medical charts, referrals, labs and superbills for HSA/FSA reimbursement and view/pay your patient statements. If you prefer to fill out the forms in our office, please arrive 10 minutes prior to your appointment time.
APPOINTMENT REMINDERS (DOOR CODE)
You will receive an appointment reminder via text (this can be changed to email or voicemail, if preferred) one business day prior to your appointment, e.g. you will receive this the Friday before Monday appointments. Look for an auto generated text from an (845) area code that will include a four digit door code that will allow you to let yourself in if you arrive extra early. Please do not respond to this (845) number; if you have any questions or need to cancel or reschedule your appointment, you can call or text our clinic mobile #: 541-972-2837 or make changes on your patient portal www.onpatient.com. If you don't receive the code, please Ring the doorbell. The front door is always locked for privacy and security reasons.
FREE PARKING
We have free parking at our office. You can use the two parking spaces labeled "reserved 150" or the three parking spaces labeled "visitor" in the parking lot behind our building on High Alley - 12th Ave between Pearl and High Street. There is also ample two hour street parking along both High Street and 12th Ave.
HOW TO PAY YOUR PATIENT STATEMENT ONLINE
This video will tell you how to view and pay your patient statements online. You can do this from our patient portal www.onpatient.com If you don’t already have a login, please text your email to our clinic mobile number and we will send you the link to the portal to create a login. Once you login, you click on the Billing tab above, and click “view” to see your statement. Now click “pay” and this will allow you to enter your credit or debit card information. FIRST APPOINTMENT
The first acupuncture appointment is 60 minutes. Your practitioner will review your intake forms and goals of treatment during the first 30 minutes, and treat you with acupuncture, reiki and/or essential oils during the remaining 30 minutes depending on what is indicated for your condition. Your practitioner will then provide a treatment plan based on your goals and his/her assessment of your condition including how many treatments you will need, over what time frame, how much it will cost and what changes you can expect from the course of treatment.
Come in, take off your shoes, put on some slippers, help yourself to some tea, coffee or water, kick back in one of our comfy recliners, soak up the sun from one of our vitamin D lamps and enjoy all of the art work. We'll be right with you!
